New Applications:
Programs applying for new certification for Level(s) 1, 2 and/or 3:
- Open and complete the application New/one-year certification (all levels - delete the levels not applicable to your need):
Stage1-ITTPC_appl_2010.doc (link to old stage1 application form)
Our new streamlined application form is now posted (instructions are on a separate web page and are under construction), 2012-Stage1-ITTPC-application - either Stage1 form is fine!
Please submit application and supporting documentation via flash drive, CD, or DVD. Emails of such large documents have proven difficult and unreliable. Mail flash drive or CD or DVD to: CRLA ITTPC c/o Rick Sheets, 12422 W. Aurora Drive, Sun City West, AZ 85375-1924

Three Year Certification Renewal Applications (for newly certified programs to renew the same level(s) of certification)
Programs applying for renewal of just certified Level(s) 1, 2 and/or 3:
- Open and complete the application for Renewal/three-year certification (all levels - delete the levels not applicable to your need):

- This application is usually a small file so email works well for it. If you wish you can instead choose to send it on flash drive, CD, or DVD.
- email: ittpc@crla.net - Click on the email link to the left to submit the application packet. It opens an email; simply type a simple message and attach the application and other needed documents. (Do not send notes or correspondence to this email, it is only for applications and the attachments!)

Five-Year Re-certification Applications are required of all CRLA ITTPC-certified programs to maintain program requirements, consistency, and integrity.
It is for programs that have previously been renewed for three years or re-certified for five years and only for the levels for which they have been previously certified. Note: If you are adding a new level, it requires a New application for certification above for that level.
