NOTE: The Stage Two application should be completed by recently-certified programs who have not lapsed at Stage One. The purpose is to reflect on and document the certification process thus far.
We recommend programs submit their application 3-to-6 months prior to the certification expiration date. Applications submitted up to six months beyond the expiration date will incur a late fee. Applications submitted beyond six months late will be required to re-apply at stage one.
Instructions:
If there are problems or missing information on your application, you will be returned to the beginning of the application with the problems that need to be corrected highlighted in red. If there are no problems with your application, you will be taken to the online payment screen.
